"Without a piece of paper, you are an insect" says the famous catch phrase. It can also be applied to cats and dogs, which also need an identity document, that is, a veterinary passport, in which all the data about the animal are entered.
Instructions
Step 1
You may need a passport if you participate in exhibitions, as well as when traveling to another city or country. Usually it is issued in clubs if you buy a purebred cat or dog. You can also get a document at any veterinary clinic. But remember that only state clinics have the right to issue veterinary certificates, while vaccinations can be done in any veterinary clinic. Therefore, in order to avoid misunderstandings, it is better to immediately contact a government agency.
Step 2
On the first page of the passport, information about the animal is usually indicated: breed, nickname, date of birth, color. All data on vaccinations, deworming and treatments of the animal are also entered here. In addition, the passport must contain a note about the chipping of your pet, if it was carried out. Fits into the document and information about the owner of the animal.
Step 3
When filling out the data on vaccination, various treatments, it is necessary to paste stickers indicating the drugs into the document, write the expiration date of the medicine, put the date, time, doctor's signature, and also a seal.
Step 4
You must fill in the reproduction box yourself. In the first column, indicate the day of the onset of estrus, then the date of mating. Next, write down the date of birth of the animals, and in the next column - the number of newborns.
Step 5
Make sure that the veterinarian fills out the document correctly. After all, a passport is the basis for obtaining a certificate for transporting an animal to another country. To obtain it, you need to provide a veterinary passport, a certificate of chipping, as well as the animal itself for inspection. At the same time, it is important to know that your dog or cat has been vaccinated against various infections no later than 30 days before your intended departure.
